Information used to respond
If you email Integrity, we use the business contact information and message you choose to provide to respond, coordinate a conversation, and maintain an appropriate business record of the inquiry.
Website operations
The hosting provider may process ordinary technical information needed to deliver and protect the website, such as IP address, device and browser information, request time, and security events. Integrity does not currently offer a public patient portal or collect patient reports through this website.
Sharing and retention
Inquiry information may be shared with authorized Integrity or Hybrid Health Systems personnel and service providers who support the response. Information is retained only as reasonably needed for the inquiry, business records, security, or legal obligations. Integrity does not sell website inquiry information.
Your choices
You may ask about, correct, or request deletion of ordinary website inquiry information by contacting Integrity. Legal or security requirements may limit a deletion request.
